Cook County Health Connect is here to make sure you have healthcare access when you need it and where you need it. Our physicians are ready to deliver the highest quality experience and care for you. Cook County Health Connect is an on-demand video visit program that allows you to connect with healthcare providers within minutes.

How Does it Work?

Download Cook County Health Connect to request a doctor for you or your family’s medical concern. You will be directed to a virtual waiting room while we pair you with a healthcare provider best suited for your needs. You will engage in a video visit from your smartphone, tablet, or computer. If your healthcare provider deems medication is needed, prescriptions may be ordered through this video visit. If an in-person appointment is considered necessary this will be communicated within your visit. The following are examples of symptoms that can be treated through Cook County Health Connect:

• Cold and Flu Symptoms
• Cough
• Diarrhea
• Fever
• Headache
• Lower back pain
• Pink eye
• Sinus problems
• Sprains and strains
• Urinary symptoms
• Vomiting

Children
• Allergy Conditions
• Cold Symptoms
• Cough
• Pink eye
• Skin rashes
• And much more

*Children under 5 aren’t eligible for video visits. Contact your pediatrician to schedule an appointment.

What Do I Need for an On-Demand Video Visit?

You need to have a computer, tablet or smartphone that has both a camera and a microphone.
You will need internet access or data service on your smartphone with a strong enough signal to conduct a video chat.
You must be in the state of Illinois at the time of the visit

If you are an Apple Health user you will have the option to share important health information with your provider during your video visit. Sharing Apple Health data is optional but allows the provider to see heart rate, weight, and other health information to help them make a more accurate diagnosis.
